Medical Reagents for USNH Okinawa

Amendment 1: The SF1449 has been updated to reflect the change from the OCD VITROS 5600 Immuno Analyzers to the OCD VITROS 7600 Immuno Analyzers, in accordance with the customer’s preference for the awarded vendor to provide this model. This update does not affect the reagents or supplies, as those for both models are interchangeable. The updated SF1449 has been added. The signature page required for submission with your offer package has not changed and remains valid. All other requirements and due dates remain the same.

Description of Requirement: Defense Logistics Agency (DLA) Troop Support Pacific has a requirement for the procurement and drop shipment deliveries of CLASS VIII, Medical supplies, materials and equipment items in support of United States Naval Hospital (USNH) Okinawa Located in Okinawa, Japan. 1.1 This is an Indefinite Delivery, Indefinite Quantity (ID/IQ) contract for Laboratory Reagents and Consumables. Individual Delivery Orders for Laboratory Reagents and Consumables medical supplies will be processed in accordance with the Delivery Schedule, located in the Attachment.

a. QUOTE Quote for items on the spreadsheets are due no later than 29 November 2024 at 8:00 am JST. All quotes shall be received using Japanese Yen. The offeror is to quote on all items for delivery to United States Naval Hospital (USNH) Okinawa Laboratory Department.

b. SF-1449 Page 1 of the SF-1449 signed and dated in Blocks 30A, 30B, and 30C.

c. LETTER OF AUTHORIZATION A letter, memorandum or certificate showing certification as a distributor of the items listed in this solicitation.

d. DELIVERY PLAN (Required only if shipping from outside Japan) A written plan explaining how delivery will be made from the vendor's facility to United States Naval Hospital (USNH) Okinawa Laboratory while maintaining temperature requirements as per the Statement of Work.
