PIMC ED & CSR (Phoenix, AZ)

This is a SOLICITATION NOTICE for the upcoming Solicitation 75H701-24-R-00035 PIMC ED & CSR (Phoenix, AZ) a construction project prepared in accordance with FAR Part 36. These construction services are in support of the Indian Health Service (IHS).

SCOPE OF WORK: The purpose of this project is to upgrade the ventilation in the Emergency Department (ED) and Fast Track waiting areas. This work will add mechanical terminal units and integrate the existing hospital automation system into them. In addition, the Central Supply Room (CSR) will have the flooring, walls, and ventilation upgraded as well. This project will require infection control mitigation.

LOCATION OF PROJECT: All work shall be performed at the Phoenix Indian Medical Center, which is located at 4212 N. 16th Street, Phoenix, AZ, 85016. PERIOD OF PERFORMANCE: The contract Period of Performance is 180 calendar days from date of notice of proceed.

CONSTRUCTION MAGNITUDE: The magnitude of this construction project is anticipated to be between $1,000,000 and $5,000,000, in accordance with FAR 36.204(f).

SITE VISIT: A site visit will be held on June 10, 2024 at 11:00 AM MST (local Time). Please confirm attendance in accordance with clause L.6. FAR 52.236-27 Site Visit (Construction) (Feb 1995) Alternate I (Feb 1995).

CONTRACT TYPE: The Indian Health Service intends to award a single Firm Fixed Price (FFP) contract in support of this requirement. This solicitation will be procured as a Request for Proposal (RFP) in accordance with FAR Part 36 procedures with Lowest Price Technically Acceptable (LPTA) evaluation criteria. This project is solicited as a 100% Small Business set-aside under North American Industry Classification System (NAICS) code 236220 Commercial and Institutional Building Construction.

The associated size standard for this procurement is $45 million. For the purposes of this procurement, a concern is considered a small business if its average annual revenue for the last five (5) years is less than $45 million. Included in this modification is an updated SF1442, updating Block 13. Amendment A00006 issued 7/20/2024 is to provide a response to pre-proposal #4, issue Addendum 02 dated July 18, 2024 which revises the Drawings Index and replaces Specification Section 230593 Testing, Adjusting, and Balancing.

This amendment also replaces Attachment J01a with Attachment J01a Specifications PIMC ED-CSR 2024.01.29 (Conformed w-Addendum 02)" incorporating theses changes. Amendment A00007 issued 7/27/2024 is to replace Attachment J02 Wage Determination: General Decision Number AZ20240039 Mod 2 dated May 17, 2024 with the attached revised Attachment J02 Wage Determination: General Decision Number AZ20240039 Mod 4 dated July 19, 2024. The proposal due date of August 7, 2024 at 2:00 PM MST remains unchanged.

Amendment A00008 issued 7/30/2024 to extend the proposal due date from August 7, 2024 to August 21, 2024 by 2:00 PM MST. Included in this modification is an updated SF1442, updating Block 13. Amendment A00009 issued 8/13/2024 to provide responses to Pre-Proposal Inquires 5-10, revise clause H.1 Work Hours, incorporate Attachments J01h-J01j and extend the proposal due date from August 21, 2024 to August 27, 2024 by 2:00 PM MST.

Included in this modification is an updated SF1442, updating Block 13. Amendment A00010 issued 8/26/2024 to replace Attachment J02 Wage Determination: General Decision Number AZ20240039 Mod 4 with the attached revised Attachment J02 Wage Determination: General Decision Number AZ20240039 Mod 5 dated August 16, 2024 and extend the proposal due date from August 27, 2024 to August 30, 2024 by 2:00 PM MST. Included in this modification is an updated SF1442, updating Block 13.
