Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Mold remediation services are required to remediate all areas of mold and water intrusion in the BioMed Area at Robert J Dole VAMC. This project will remediate the mold present in the basement of building 2 in BioMed.

This will include the proper containment, removal, cleaning, and reconstruction of walls, bases, painting, and ceilings as required to remediate and rebuild approximately 3 affected areas throughout the BioMed area is currently estimated to be $30,950.86. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The main driver for one source is due to the emergency nature of the issue.

We must have contractors on site immediately to start this work. This is due to the seriousness of patient and employee safety in preventing sickness caused by the presence of mold. The vendor we have chosen has completed work here recently along the same lines with mold remediation. They can do the work, and are local.

Description of market research conducted and results or statement why it was not conducted: Floyd Construction Corp. though has completed this type of work at the Robert J Dole VAMC as a contractor in the past and has the capabilities to perform the needed work to remediate all areas of mold and water intrusion.

Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
