What is the process for tracking these loaner instruments within the hospital to make sure they are returned to the contractor when the repaired instrument is returned to the facility? 4. Page 8 Item o. Is providing new replacement instrumentation a requirement? If contractor can make recommendations for instrument replacement, and can provide new instrumentation, but not at the time of repair, does this disqualify their bid?

Page 10, Sterilizer Cleaning: You are asking for sterilizer cleaning, but the request also indicates your expectation is that the repair vendor must also provide service maintenance for the units, is that correct? 6. Clarksburg shows 0 autoclaves in CLIN 0008, does that mean there are no autoclaves requiring cleaning there? 7. Washington has 20 autoclaves in CLIN 0012, does that mean there are 20 to be serviced at each quarterly visit, or 5 at each quarterly visit?
