This amendment is to make the following changes: -Incorporate Questions and Answers for RFQ W912PQ-24-Q-0015 -Incorporate changes to the PWS, sections 5.19 -Change/update the line item structure to reflect price per test, not full and half day quantitates -To extend the solicitation to 3pm EST on 12 August 2024 This solicitation is issued as a Request for Quote (RFQ) from the USP&FO for New York, Purchasing and Contracting Division, Latham, New York.

The solicitation number is W912PQ-24-Q-0015 for the procurement hearing tests throughout NYS per the solicitation and PWS. Principle NAICS is 621340 and size standard is $12,500,000. This acquisition is utilizing procedures in FAR part 13.5, Simplified Acquisition Procedures. This requirement is set aside 100% for Small Businesses. 52.204-26 MUST be included with the quote, if not in sam.gov registration.

Award will be made to the responsible offeror registered as a Small Business in Sam.Gov, whose quote/offer, conforms to this Request For Quotations, and is the lowest evaluated offer/quote, based on Price alone. Quotes shall include all necessary licensing information required in the Performance Work Statement (PWS). Offeror/Quote must meet all the responsibility criteria at FAR 9.104. Award will be made without discussions. These quotes shall conform to the requirements of 52.212-1.

The Government will not accept partial quotes/offers. Award will be made without discussions. Quotes are due 12AUG2024 1500 via email to johanna.a.goman.civ@army.mil AND melissa.santoro2.civ@army.mil . In order to be eligible for award, the successful offeror's System for Award Management (SAM) (https://www.sam.gov) registration must be active and includes representations and certifications.
