Amendment 0002 - The purpose of this amendment is to extend the quote due date.

Amendment 0001 - The purpose of this amendment is to add the CPT cod list to the solicitation.

The New York Army National Guard has a requirement for Laboratory Services. To provide all labor, materials, and services to perform the requirements as stated in the attached Performance Work Statement. Utilize the incorporated Estimated Test Per Location Breakdown sheet to formulate your quote. Please include per location prices with the estimated tests per location and the cost to complete all the estimated test for the entire base year.

Please complete and submit one sheet for the base year, option year 1, and option year 2. This sheet is incorporated in the RFQ and is included in the SAM.gov posting for convivence. Basis of award will be the lowest price that meets all the requirements of the Solicitation and Performance Work Statement. Wage Determination 2015-4143 Revision 26 dated 26 December 2023 applies and is incorporated. Please sign and return a copy of the RFQ and Amendment(s).
