The offeror is registered in SAM with no active exclusions. The contractor has provided documentation of training and certification in Confined Space Entry and Rescue at the time of Quote. The contractor has provided a valid State and/or Federal Photo Identification for each team member at the time of Quote. Offerors should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services with their Quote.
