That attachment must be completed and returned to be considered, a YES or NO should be selected beside every requirement on spreadsheet. You must also submit the following documents to be considered for this solicitation: Toxicity Lab report, Your USDA Bio preferred certification letter from the USDA for the exact product you are offering, A SDS for the exact product name you are offering, a quote on company letterhead with the total price of bid, it should list the quantity and acknowledge you are providing all of the required equipment. These documents are listed again in the meets or exceeds spreadsheet.
