Submission of any information in response to this market research is purely voluntary. Contractors need to meet the requirements of the Source Approval Program including vehicle level qualification testing, at their own expense. The TACOM Life Cycle Management Command, Competition Management Office (CMO), is the lead proponent for the Source Approval Program.

Full name and address of the firm 2. Unique Entity ID and CAGE Code 3. Point of Contact 4. Size Status (i.e., Small Business, 8(a), HUBZone, Service Disabled Veteran Owned Small Business (SDVOSB), Small Disadvantage Business, Women- Owned Small Business) 5.
