1. Description of Requirement: Defense Logistics Agency (DLA) Troop Support Pacific has a requirement for the procurement and drop shipment deliveries of CLASS VIII, Medical supplies, materials and equipment items in support of United States Naval Hospital (USNH), located in Okinawa, Japan. 1.1 This is an Indefinite Delivery, Indefinite Quantity (ID/IQ) contract for Laboratory Reagents and Consumables.

Individual Delivery Orders for Laboratory Consumables medical supplies will be processed in accordance with the Delivery Schedule, located in the Attachment a. Quote for items on the spreadsheets are due no later than November 13, 2024, at 15:00pm Japan Standard Time (JST). All quotes shall be received using Japanese Yen.

The offeror is to quote on all items for delivery to United States Naval Hospital (USNH) Okinawa, Camp Foster Laboratory. b. SF-1449 Page 1 of the SF-1449 signed and dated in Blocks 30A, 30B, and 30C. c. LETTER OF AUTHORIZATION A letter, memorandum or certificate showing certification as a distributor of the items listed in this solicitation. d. DELIVERY PLAN (Required only if shipping from outside Japan) A written plan explaining how delivery will be made from the vendor's facility to United States Naval Hospital (USNH) Okinawa, Camp Foster Laboratory while maintaining temperature requirements as per the Statement of Work.
