1/15: This is an amendment for this requirement. to change the quote due date and to set the minimum requirement for the partial shipping delivery.

NOTE: Read all paperwork carefully for compliance and pay special attention to the NAICS Code being used. The NAICS Code must be listed in your SAM profile. This is a combined synopsis/solicitation for 1500 CFM HEPA Filters.

1. Make sure there is a Point of Contact Name, Email, and Phone Number on your submittal.

2. RETURN ALL paperwork requested (RFQ-filled out completely & any attached forms). The PSNS RFQ form AND Vendor Information sheet is required. An internal Company quote is optional.

3. Annotate in the subject line the amended solicitation number- N4523A26Q4003-0002 HEPA Filters See the attached RFQ and other documents for details of the requirement and submittal instructions.
