The Indian Health Service, Kayenta Health Center located at US Highway 160 South Milepost 394.3, Kayenta, AZ 86033, has an upcoming requirement for three hazardous pre-fabricated mobile storage units with a delivery within 30 days upon receipt of purchase order.  The North American Industry Classification System or NAICS code is 332311 Prefabricated Metal Building and Component Manufacturing with a small business size standard of 750. 

The Request for Quotations No. 75H71026Q00004 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 09/24/2025.  Evaluation factors may include technical capability to meet the government needs IAW the statement of work, past performance, and delivery.  Quotes will be due on or by 09/30/2025.  The acquisition is a small business set aside.

In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
