The Indian Health Service, Northern Navajo Medical Center, located in/at Hwy 491 N, Shiprock, New Mexico 87420, and Dzilth-na-o-dith-hle Health Center, located in/at 6 Road 7586 Bloomfield, NM 87413 has an upcoming requirement for two (2) Optometrists to perform on-site services to the eligible beneficiaries of the Indian Health Service. The North American Industry Classification System or NAICS code is 621320 Office of Optometrists with a small business size standard of $9.0 million.

The Request for Quotations No. 75H71024Q00057 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Procedures for Certain Commercial Products and Commercial Services will be issued on or by March 25, 2024. Evaluation factors may include technical capability to meet the government need, minimum education requirements, professional certifications, past performance, and price. Quotes will be due on or by April 12, 2024, by 2:30 p.m. MDT.

The acquisition is a total 100% Indian Small Business Economic Enterprise set aside.
