Fisher Medical and Lab supplies

REQUIREMENTS: Indian Health Service (IHS) – Phoenix Area Office (PAO), 40 North Central Ave., Phoenix, Arizona 85004-4424 has the requirement: General Fisher Medical Supplies. (See enclosed list) - This list is not all inclusive. A purchase order/delivery order/blanket purchase agreement contract will be awarded in a lump sum dollar amount for a base year and 4 option years and supplies and orders will be placed against the dollar amount. A contract modification will be issued if the dollar threshold is exceeded.

Once the REQUEST FOR QUOTE period has ended – The award basis will be made evaluating vendor quote against the 60 items listed on enclosed supply listing and evaluation of the service unit. This is currently a 100% set-aside for small businesses on SAM.GOV. Delivery will be to multiple locations.
