Modification - extension to 9/11/2024, change in clause The Indian Health Service, Shiprock Service Unit, Northern Navajo Medical Center, located at US Hwy 491 North, Shiprock, New Mexico 87420 has a requirement to conduct survey data for Ambulatory Care and Emergency Medicine for the Hospital Consumer Assessment of Healthcare Providers and systems. Services must be performed in accordance with the attached SOW.

Evaluation factors may include technical capability to meet the government need outlined in the statement of work, past performance, and price. Quotes will be due on or by September 11, 2024.
