Audio Hazard Screening Services

Amendment #2 8-5-2026: The purpose of this amendment is to update the solicitation closing date until 12 August 2026 at 1:00 PM EST. Amendment#1 7-30-2026: The purpose of this amendment is to post a consolidated list of questions and answers. This solicitation will result in a Blanket Purchase Agreement with a 5-year ordering period. The Contractor shall provide health care support for Blanchfield Army Community Hospital.

The contractor shall perform Audiology evaluations for eligible Department of the Army civilian employees from Rock Island Arsenal, Illinois (RIA) referred from the Occupational Health Nurse, RIA. The contractor shall provide the contract services at the contractor’s office, which must be located within a 60-mile radius of RIA as a required condition for effecting the contract services.

These services will be in support of, as needed, audiometric testing, evaluation, and remedial services for patients with a variety of hearing problems, meeting or exceeding current recognized national standards as established by the Joint Commission and the American Medical Association (AMA). The services will be of a nature and scope described in the paragraphs below. The Contractor shall provide the services of Audiologist licensed to perform services of the nature and scope described in this Performance Work Statement (PWS).
