AMENDMENT 002 EXTENSION: The purpose of this amendment is to:

1. Correct Depth of mattresses from 10.5 inches to 9 inches.

2. Extended response due date from 1 July 2025 at 10:00AM CDT to 3 July 2025 at 10:00AM CDT 3. Upload RFQ Questions and Answers for the Updated Quantity of mattresses (800) Please see updated attachments "Solicitation Amendment - FA301625Q01170002 Extension" and "RFQ Questions and Answers for JBSA Lackland Mattresses 800 quantity".

AMENDMENT 0002: The purpose of this amendment is to:

1. Extended response due date from 20 June 2025 at 10:00AM CDT to 1 July 2025 at 10:00AM CDT 2. Updated quantity of mattresses from 400 twin XL mattresses to 800 twin XL mattresses. 3. Updated delivery address to add Bldg. 127, 1040 Apollo Dr, JBSA Lackland, TX 78236. 4. Statement of Work edited to reflect updated quantity of mattresses and updated delivery addresses Please see updated attachments "Solicitation Amendment - FA301625Q01170002" and "23 June - Mattress SOW".

AMENDMENT 0001: This RFQ edit is to incorporate the "RFQ Questions and Answers" document.

This is a combined synopsis/ solicitation for the delivery, installation, removal, and disposal of 400 twin XL mattresses. This combined synopsis/ solicitation for a commercial product is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6. The solicitation is issued as a Request for Quotation (RFQ). This service is a 100% Total Small Business Set Aside only and the associated NAICS Code for this requirement is:

33790 - Mattress Manufacturing The vendor shall respond to this RFQ using the attached Solicitation document and send it via email to Christina Shoffner, email: christina.shoffner@us.af.mil and N.
