Combined Synopsis/Solicitation - CDC Sole Source to National Association of School Nurses for SBAS - Phase 3

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued.

The solicitation number for this requirement is 75D301-24-Q-77965_CDC_NASN-SBAS Phase 3 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The purpose of this contract is to build on the work from Contract # 75D30121C12121 for the school-based active surveillance among school children. As such, CDC is proceeding with a sole source award to The National Association of School Nurses (NASN) for Phase 3 of the School-Based Active Surveillance (SBAS).

The Centers for Disease Control and Prevention intends to award a firm-fixed-price, non-severable service contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: ITEM / SUPPLIES - SERVICES / QTY - UNIT / UNIT PRICE - EXTENDED PRICE 0001 / NASN - SBAS Project Phase 3 / 1 Job Focus on ME/CFS Task 1 - 7 07/26/2024 - 09/25/2025 Non-Severable Services (All work shall be performed in accordance with the attached 75D301-24-Q-77965_NASN-SBAS Phase 3_Statement of Work (SOW)) Your quote shall include the following:

This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2024-05 (May 22, 2024). This acquisition is under North American Industry Classification System (NAICS) code 813920 with small business size standard $23.5M. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management.
