IVANTIA Maintenance

Request for Quotation (RFQ) 472866 IVANTI Maintenance December 04, 2025 Brookhaven Science Associates, LLC (BSA) operator of Brookhaven National Laboratory (BNL) under a Prime Contract with the U.S. Department of Energy (DOE) seeks to procure IVANTI Maintenance. Requirements for this solicitation are outlined in the attached RFQ Document. Ouote package shall include the below:

The Offeror shall complete and return the attached Enclosure C. Quotation Pricing Sheet. Pricing shall remain firm for sixty (60) days to allow time for review, selection, and Award. Prices shall be quoted in U.S. Dollars.

Other Submittals: SAM Unique Entity Identification (UEI) Number within attached Enclosure C. Quotation Pricing Sheet, as well as affirm Representations and Certifications and size status are accurate and up to date in SAM. *Foreign Offerors are not required to be registered in SAM; however, must fill out the attached Enclosure B. AMS-Form-050 (Annual Representations and Certifications) if they are not.

The attached Enclosure B. AMS-Form-050 (Annual Representations and Certifications) is only required for Offerors that are not currently registered in SAM. A current copy (less than 12 months prior to the date of solicitation) of Enclosure B. AMS-Form-50 (Annual Representations and Certifications) must be on file with BSA's Procurement and Property Management (PPM) Division before an Award can be placed. The successful Offeror will be required to complete new vendor forms, if applicable.

CRITICAL DATES Questions Due Date: Monday, December 8, 2025, at 4:00PM Eastern Standard Time Quote Due Date: Wednesday, December 10, 2025, at 4:00PM Eastern Standard Time Anticipated Period of Performance: December 15, 2025 through December 14, 2026. * BSA is requesting two (2) additional option years that are exercisable in one (1) year increments at the unilateral discretion of BSA. The options, if exercised, will be executed by written Change Order at unit pricing agreed to at time of award.
