Continuation of SF-1449, Block 3: The following information shall be entered by the Contracting Officer at the time of contract award according to the timeframes specified in this solicitation s Section B, Scope of Contract, Section 1.4 Contract Effective Date, Implementation Period, and Ordering Period.

Offerors shall not enter information into the above fields. Continuation of SF-1449, Blocks 19 through 24: To be considered for award, offerors must submit a price for line item 1A, or 1B, or 1C for the base year and all four option years. One award will be made for line item 1A, or 1B, or 1C for the base year and all four option years. Offered prices shall not exceed two decimal places.

The price evaluation will be completed by multiplying the proposed BTK Inhibitors (Ibrutinib/Acalabrutinib/Zanubrutinib) price by the number of units per day required per patient to calculate a cost per day. This cost per day will then be multiplied by the estimated number of patients (a total of 2,444 for all participating agencies) for the proposed product and 365 (days per year) to calculate a total cost per year.

The required number of units per patient per day are listed in the table for each item (1A, 1B, 1C) in the Schedule of Supplies and Attachment B of this solicitation. This method of price evaluation will ensure that the prices of proposed products can evaluated accurately and equitably. Unit prices offered shall not exceed two decimal places. The Government intends to evaluate offers and award a contract without discussions with offerors.

However, the Government reserves the right to conduct discussions if determined by the Contracting Officer to be necessary. The awarded product will be the only agent listed on the VA National Formulary for Chronic Lymphocytic Leukemia (CLL) and Small Lymphocytic Lymphoma (SLL) indications and will required be for new patient starts. The offeror must make all package sizes for the eligible strengths available. All proposed/awarded package sizes will be awarded at the same price per capsule/tablet.
