Statement of Work Par Excellence replacement equipment VA Black Hills Health Care System with a site location in Hot Springs 500 North Fifth Street, Hot Springs, SD 57747 here in after referred to as the Hot Springs VA, needs a onetime purchase for Par Excellence equipment. The equipment provided must be new and required to be fully operational and without defect. On December 7, 2023, there was a fire on the Hot Springs logistics loading dock.

The fired destroyed Par Excellence scales, bins, wiring, louver panels, and scales. The required items requested below are what needs to be replaced from current SDVOSB vendor (SDV Medical). The current project is at a standstill until the required equipment is replaced.

A. Delivery: Par Excellence equipment shall be shipped upon receipt of award. Deliveries shall be made to:

Hot Springs VA Warehouse (6), Ref. PO#:_499A40000_ 500 North Fifth Street Hot Springs, SD 57747 Delivery times are between 8am and 3pm Monday- Friday, except Federal Holidays.
