STATEMENT OF WORK (SOW) 1. Contract Title. Straker Battery Hydraulic System Installation. 2. Background. The Department of Veterans Affairs requires the installation of three the Battery-Powered Hydraulic System, Straker Model #6390, Product# 63900555001. The Straker Model #6390, Product# 63900555001cot/gurney fastener system lifts and lowers the cot into and out of the ambulance, reducing spinal l trauma injuries.

The Straker Model #6390, Product# 63900555001 cots/gurney fastener system improves operator and patient safety by supporting the cot/gurney throughout the loading and unloading process. 3. Scope. The contractor shall install Straker Model #6390 with Product# 63900555001 on VA premise only. The installation of the Straker Model #6390, Product# 63900555001 shall only be installed on site. This will reduce disruptions of our patient care and services our Veterans. 4. Specific Tasks.

Install three (3) Battery Hydraulic System Straker Model #6390, Product# 63900555001 installed on the VA premise only. 5. Performance Monitoring N/A 6. Security Requirements N/A 7. Risk Control Using a Battery-Powered Hydraulic System reduces trauma injuries to the EMT s staff and improve staff and patient safety. 8. Place of Performance. The install will be completed at the DEPARTMENT OF VETERANS AFFAIRS Greater Los Angeles Healthcare System.

Location: 11301 Wilshire Boulevard Los Angeles, CA 90073. 10. Delivery Schedule. West Los Angeles Healthcare Medical Center will schedule the installation date.
