1/29/24:

Amendment 0005 and SOW Rev 1.29.24 attached to provide clarifcations for the Patient Entertainment System. The Carl T. Hayden Medical Center located at 650 E. Indian School Road, Phoenix, AZ 85012 has a requirement renovate the Community Living Center located within the first and second floor of Building 16. The intent of this project will modernize the facility rooms and halls. Work will be performed in a functioning hospital environment.

The contractor shall furnish all supervision, tools, labor, materials, equipment, and transportation necessary to accomplish the work. All work shall be performed in strict accordance with the terms, conditions, specifications, and drawings (when available) of the solicitation and all applicable local and VA building codes.

This project will be procured as 100% set-aside for Service-Disabled Veteran-Owned Small Business (SDVOSB) and must be listed as verified in the Small Business Administration (SBA) VetCert database at https://veterans.certify.sba.gov/. The North American Industry Classification Code (NAICS) used for this requirement is 236220, Commercial and Institutional Building Construction with a Size Standard of $45.0M.
