Combined Synopsis/Solicitation Notice Combined Synopsis/Solicitation Notice Page 3 of 3 This is a combined synopsis/solicitation for two (2) (BD) Site Rite 9 Ultrasound equipment and accessories as shown in the attached SF1449 RFQ solicitation B.2 for the Seattle-Puget Sound WA, VA Medical Center. This is for BRAND NAME ONLY, as prepared in accordance with the format in Subpart 12.6, 13.106-3 and 13.501 as supplemented in the attached Request for Quotes (RFQ). A firm-fixed price purchase order is anticipated.

Simplified Acquisition Procedures IAW FAR 13 will be used. (ii) The solicitation number is 36C26024Q0899 and is attached. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-06 effective 07/30/2024. (iv) This solicitation is set aside 100% for certified SDVOSB only and listed at this site: https://veterans.certify.sba.gov/ and shall be authorized sellers or distributers of the manufacturer and provide proof of this authorization with their quote.

The associated NAICS 339112 code has a small business size standard of 1000 employees. The Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.

The vendor shall provide new items ONLY; no remanufactured or "Gray Market" items. All items must be covered by the manufacturer's warranty. Installation is required by local manufacturer area manager. (v) Delivery shall be within 30 days from the time of award and acceptance and shall be delivered to:
