RFQ NO. 36C25924Q0523 - FIRE/SMOKE DOOR INSPECTION & LABELING AMENDMENT NO. 0003 JUNE 26, 2024 Purpose for Amendment: Extending responses due date: FROM June 26, 2024 TO June 28, 2024 Posting Questions & Answers (Q&A) Change estimated period of performance: FROM June 30, 2024 October 28, 2024 TO July 15, 2024 November 12, 2024 QUESTIONS AND ANSWERS Question 1. Classroom Session & Hands On Training: Is a training certificate required following the training?

VA Response: Yes. Training itinerary and certificate is required. This is to fulfill a TJC requirement. The intent of training is to provide qualified and certified staff to perform inspections. Question 2. UL List Fire Smoke Assembly (200 qty): Add opening identification stickers for UL Rated Assembly Can you speak in greater detail to what the VA is looking for here? How was the quantity of 200 determined/confirmed?

VA Response: The VA has several doors without fire rating labels. We are looking to get the 200 doors rated as listed assemblies to meet a TJC requirement. The count is from previous inspections made in-house. Question 3. Re-Inspect & UL Label Assembly (55 qty): Re-inspection and UL Labeling Can you please provide additional information regarding where the quantity of 55 comes from? Does this require 55 doors to be re-inspected and an additional 55 doors to be relabeled? Or is it 55 total for both re-inspection & re-labeling?

VA Response: It is estimated that 55 doors will fail initial inspection and need to be reinspected. This is an estimation based on the total number of doors. Question 4. In the scope of work, we have two separate sections for labeling ( Add opening identification sicker for UL rated assembly) and ( Re-inspection and UL labeling) Could you kindly provide us with more detail about what the distinguishing difference is between the two?

VA Response: During the initial inspection the doors will fail due to no label. The doors will need to be labeled and reinspected to gain a passed inspection. Some doors could require a repair, then labeled to gain a passing inspection.

Question 5. Contract Line-items:

0003 UL List Fire Smoke Assembly:                                           i.    Is there an inspection/testing requirement for these 200 openings that are going to receive these stickers ?                                          ii.    Are these 200 openings in addition to the 725 from line 0001?                                         iii.    Who is providing the sticker design and is there any customer requirements if we provide them?

0004: Says to Re-inspect and UL Label                                           i.    Is that to provide up to 55 Fire Rated Field Labels?                                          ii.    Since we are not part of UL, may we supply and apply our own PCS Field Label? Proposing firm is ISO/ANAB accredited to Field Label VA Response: Doors will need to maintain UL listed assembly per SOW.

Question 6. 3.3 Resources Will we receive current Life Safety Drawings to ascertain location of rated openings and also rating of wall and opening protective?

VA Response: Yes. All doors are located at 1700 Wheeling St. Current Life Safety drawings will be provided as well as an escort for access.

Question 7: 3.9 Contractor a. AAADM has nothing to do with inspection and testing of fire rated door openings. Is it really a requirement to have an automatic door tech inspect fire rated and/or smoke rated door assemblies when they are so different? i. Proposing firm uses training for NFPA 80 (fire) and NFPA 105 (smoke) NOT AAADM for this type of inspection/test VA Response: NFPA 80 and NFPA 105 training for Fire and Smoke rated assemblies is acceptable. AAADM would only cover the closure rate and adjustment for the doors.

Question 8: Under 2.0 Scope a. When the solicitation states a location of Campus-wide , how many buildings is that, and how far apart are they? b. Under 2. What does BLD L/Field mean? i. Are those different classes?

VA Response: The campus is located on 34 acres with 13 buildings. Building occupancy varies from patient care to business with several different uses. BLD L is Building L and field refers to at location such as field verify.

Question 9: 3.0 General a. Uses the term service the equipment, as required . i. Do you expect repair or parts pricing with this RFQ or are all repairs of deficiencies treated as Change Orders?

VA Response: Service for the equipment as required is to be included in the total contract price. The intent of this solicitation is to have all doors with a passing inspection when period of performance and SOW is complete.

Question 10: 6. 3.5.1 a. Uses the term routine repair . i. Same question as my #5 above. Any repair or parts pricing with this RFQ?

VA Response: Same as response above. Routine repairs are to be included in the total contract price. The intent of this solicitation is to have all doors with a passing inspection when period of performance and SOW is complete. Question 11. Do DBA or SCA rates apply for this opportunity?

VA Response: DBA does not apply. SCA does apply, as indicated by the Wage Determination referenced on page 14 of the RFQ.
