Surgical Lights for Operating Room

Amendment 0003: The purpose of this amendment is solely to correct an error found on amendment 0002. In the purpose of the amendment section found on page 2 of amendment 0002, it incorrectly stated the RFQ submission due date is 09 SEP 2026. The correct due date for RFQ submission is 04 SEP 2026 at 12:00pm CST. There are no further changes with this amendment.

Amendment 0002: The purpose of this amendment is to:

1. Provide the Government’s answers to the questions submitted. See Document 36C25626Q1212 0002 2. Extend the due date for RFG submissions to 12:00pm CST on 09 SEP 2026. See attached document: VHA ICRA-1.4 April 14 2026.pdf (Not to be completed for quote submission, but for reference.) For you quote submission, please complete section B.1(5) “Acknowledgement of Amendments” of the solicitation under Contract Administration Data, acknowledging both amendments (0001 and 0002).

Amendment 0001: The purpose of this amendment is to:

1. Provide pictures of the 5 rooms for installation, including the CAD PDF drawing of the area with rooms labeled. See Amendment Attachments.

2. Confirm there will not be a site-visit for this requirement.

3. Address Questions. Due to the volume of questions received, we will officially close the period for questions today. Please ensure all questions are submitted no later COB 08-21-2026. An additional amendment will be posted answering all questions received early next week.

4. Extend the due date for the RFQ submissions from 12:00pm 26 AUG 2026 to 3:00pm, 02 SEP 2026 to allow vendors additional time to review Government responses to RFQ questions.

Original Solicitaiton: This solicitation is a request for quote (RFQ) for surgical lighting with delivery and installation for the MEDVAMC in Houston, TX. The Government intends to award a firm-fixed-price contract. See B.2 Price/Cost Schedule/B.5 Statement of Work Pages 2 through 28 for RFQ details. See Addendum to 52.212-1 Instructions to Offerors for Instructions for proposal submission pages 30 through 35. This requirement is a Total SDVOSB Set-Aside. This solicitation is in accordance with RFO 12.201-1.
