This is not a solicitation and this notice shall not be construed as a commitment by the Government to issue a solicitation or award a contract. Any information received as a result of this sources sought does not restrict the Government to any type of solicitation, if one is issued. Description of Requirements: Government is seeking contractor to provide Community Nursing Home (CNH) Services based on Medicaid RUG rates as an all-inclusive per diem rate in Shreveport, Louisiana and surrounding parishes listed below:

Bienville Parish Bossier Parish Claiborne Parish De Soto Parish Jackson Parish Lincoln Parish Ouachita Parish Sabine Parish Union Parish Webster Parish Responsible sources are encouraged to submit a response to this notice with a statement of interest on a company letterhead.

At a minimum, the following information shall be provided: 1. Company Name 2. Company Mailing Address 3. Point(s) of Contact including telephone number(s) & email address(es) 4. Socio-Economic (i.e. Small/Large Business, HUBZone, Service-Disabled Veteran Owned, 8(a), etc.) as it relates to NAICS Code 623110 Nursing Care Facilities (Skilled Nursing Facilities) ($30 Million). 5. Provide a summary of the type of services that your CNH facility provides.

6. Company UEI number and confirmation that company is registered in the System for Award Management (SAM), www.sam.gov. It is the intent of the Government to issue a competitive multiple award Indefinite Delivery Indefinite Quantity (IDIQ) type solicitation after all market research has been completed.
