578-26-1-6098-0001 Project: 578-M26110 SPS Repairs after wastewater leak Contracting Agency: Great Lakes Acquisition Center (GLAC) 2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Facility has experienced damage in room A022 & A025 which is their Sterile processing decontamination and sterile processing prep workspaces. They were damaged due to a wastewater piping breaking in the ceiling.
Unfortunately, everything in the workspace has been deemed nonusable and needs to be repaired. Therefore, work includes repair/replacement of all ceiling tiles in that area, repair/ replacement of fluorescent lighting with LED lighting, and repair/replacement of approximately 70 feet of cast iron piping with hangers and fittings. It is imperative that this project proceeds without further delays to support veteran care.
Therefore, the CO determined to sole-source this project to the recommend WOSB that was recommended. The resultant award will be a stand-alone, firm fixed price, construction contract. The period of performance will be 180 calendar days upon notice to proceed.
The estimated value of this procurement is [REDACTED] 3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Unusual and Compelling Urgency per 41 USC §3304(a)(2) as implemented by FAR 6.302-2. An award will be made to a woman owned small business (WOSB) demonstrating the ability to promptly mobilize in this emergency.
The Hines VA cannot risk potentially more damage to that area, the hospital, or its employees, while waiting to solicit proposals for this project. 4. Description of market research conducted and results or statement why it was not conducted: This action shall be procured as a sole source for a new contract at VA Edward Hines Jr. VA Hospital in Hines, Illinois. Station 578 currently holds no existing contract with the prospective Contractor.
The Contract Specialist (CS) conducted market research of the Small Business Administration (SBA) to verify the contractor's active status, which identified the contractor as a Woman-Owned Small Business (WOSB). Market research was also conducted via performed the System for Award VHAPG Part 813.106 Simplified Acquisition Procedures:
Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT Management (SAM). The CS verified that Pro Fire and Water Restoration Company is a WOSB and is active in SAM (not debarred or suspended). The market research was completed after the emergent NTP/ NTE was issued and will be used for future references. No additional market research was conducted on this action due to the urgent nature of this procurement.
5. Contracting Officer's Certification: I certify that the requirement outlined in this justification is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.
