BRIEF DESCRIPTION: THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL SERVICES PREPARED IN ACCORDANCE WITH THE REVOLUTIONARY FAR OVERHAUL (RFO) IN FEDERAL ACQUISITION REGULATION (FAR) SUBPART 12.202, “STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES” AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED.

PERIOD OF PERFORMANCE ESTIMATION: Base +2 potential Option Years. Estimate start date 9/7/2026-9/6/2027 base year.

PLACE OF PERFORMANCE: Louise Stokes VAMC And VA Outpatient Surgery Center 10701 East Blvd 8901 Superior Ave. Cleveland, Ohio 44106 Cleveland, Ohio 44106 NAICS CODE AND SMALL BUSINESS SIZE STANDARD: 541380 $19 million. If you feel another NAICS code could apply, please present that information with your package.

SET-ASIDE STATUS: This RFQ is set-aside 100% for Small Business Responses.

INSTRUCTION FOR QUOTERS: Email complete package and communication to Jennifer.Nowak@va.gov Contract Specialist before deadlines. Ensure that you fill out B.1 with your name, address, SAM UEI #, contact information and sign the SF1449 at the bottom. Read the document in its entirety including any and all attachments. Complete the price/cost schedule within the document or provide a quote for all line items listed in that section.

Any questions must be emailed to Jennifer.Nowak@.va.gov and the cut off for questions is 8/31/2026 by 1 pm EST. Submission of your completed RFQ response is due by 8/4/2026 at 12:00 pm EST. STE108 is a compliance assessment tool VHA Directive 1116 is the policy it measures against. Attachments have been included.

SITE VISIT: There will be no site visit for this RFQ and/or acquisition. This is for testing as shown in this SOW.
