SOURCES SOUGHT NOTICE For Onsite Board-Certified Cardiac Perfusionist to in on-call for the VA Ann Arbor Healthcare System in Ann Arbor, MI. This is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. A request for quotes may be issued at a later date.

This Sources Sought/Market Survey Notice is being conducted by the Network 10 Contracting Office (NCO 10), PO Box 492, Ann Arbor, MI 48106 to identify sources that can provide on-site Board-Certified Cardiac Perfusionist to include on-call Services. Board eligible is not accepted. The contract perfusionist shall have a minimum of five (5) years of experience and be expected to operate the cardiopulmonary bypass machine for cardio cases any day they are onsite.

The contract perfusionist will be expected to work a tour of duty no less than eight (8) hours when scheduled and must stay until completion of the open-heart case for the day, whichever is later. For on-call services, the contractor is to respond within an hour of request and provide a board-certified perfusionist with a need of eight (8) hours or less. Perfusionists shall be onsite and available during all scheduled OR hours and within 30 minutes of the facility.

The contract candidate will meet all VA employee requirements including background check and drug testing. Perfusionist shall maintain certification throughout the administration of the contract. The contractor shall provide at minimum two candidates that meet all the above criteria.

Period of Performance: Five (1) one (1) year ordering periods that will begin from effective date of Award through the end of the five-year ordering period.

Hours of Service: VA business hours/hours of operation are Monday through Friday, 7:00 a.m. to 3:30 p.m., excluding federal holidays. Responses shall include:

(1) Business Name, GSA contract number and Address (if applicable) (2) Point of Contact Name, Phone Number, and E-mail Address (3) SAM UEI and NAICS code (4) Business Size SMALL or LARGE (5) Type of Business SERVICE-DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUB Zone, WOMEN-OWNED etc. All businesses eligible to provide this service are encouraged to reply as this information may be used to determine potential set asides for the above noted service.
