1. OFFER SUBMISSION Offeror shall submit their offer via email to Alexis Alexander at Alexis.Alexander1@va.gov by the due date/time identified in Block 8 of the SF1449. Submission shall include solicitation identifier 36C25024Q0228 to ensure receipt of offer. Offers shall be assembled by the following Sections; each section shall be easily identified. Section A - Offer Form (Standard Form 1449) Shall be signed and dated.

Data Universal Numbering System (DUNS) Number shall be entered in block 17a. with the Offeror s name and address. Section B Contract Administration & Pricing Fill in the offeror s Company Name, Point of Contact (POC) along with their email address and phone number at B.1 All line items shall be filled out per the Price/Cost Schedule at B.2. All pricing shall be in accordance with the Statement of Work at B.3 The offeror s technical quote shall address how they intend to meet all SOW requirements.

Section C Contract Clauses The technical quote must address how the offeror plans to meet the VAAR requirements in the following clauses: VAAR 852.219-75 please confirm understanding of and agreement with completing the certification portion of the clause. Section D Attachments The technical quote must acknowledge the applicability of the Wage Determination All pricing shall be offered in consideration of applicable wage determination at section D.1. and prevailing wages of any applicable job classification.
