RFI QUESTIONS AND QUESTIONS:

1st Vendor: I respectfully request any available photographs of the existing header currently installed. Attached If photographs are not available, we would greatly appreciate any identifying information you may have for the existing header, including the manufacturer, model number, and part number. This information will assist us in accurately evaluating the existing conditions and preparing our quotation accordingly.

2nd Vendor: 1. Is there a proposed site visit? Not currently planned 2. Can you provide the most recent annual medical gas inspection report? Attached 3. Can you provide pictures of the backup oxygen header? Attached 4. Is the backup oxygen header outside at the bulk oxygen site or an in-building emergency reserve? In-building reserve 5. Can you provide pictures of the valves that are being replaced? Attached.. Although some of the missing zone valves either could not be located or do not exist.

6. Can you provide sizes of the valves being replaced? Attached.. Although some of the missing zone valves either could not be located or do not exist. 7. Can you confirm you are referring to zone valve boxes (in hallways, operable from a standing position) or are you referring to service/in-line valves (in ceilings, mechanical rooms, etc.)? Unknown, some of the missing zone valves either could not be located or do not exist.

8. Can you clarify the zone valve issue; are there zone valves missing that need to be added, or are we replacing leaking zone valves? SOW says, missing zone valves , but it seems unlikely that the facility is missing that many valves. Zone valves have been in the code for decades. Some of the missing zone valves either could not be located or do not exist. 9. SOW (B) states 26 zone valves; is it 26 different locations or 26 total valves.

Ex: Oxygen, Medical Air, Vacuum for ICU. In your count, are you counting that as 3 valves even though it s in a single zone valve box . See attached report for details. 10. Can you identify the areas where the new zone valves will be installed? OR s, ICU, PACU, Radiology, etc. The area they are in impacts the ICRA procedures and hours of operation. Patient exam rooms. ICRA procedures will likely be required.

3rd Vendor: Please let me know if we can schedule a site visit so we can gain a better understanding of the scope of work and provide a more accurate quote. Not currently planned 4th Vendor: Will there be a site visit made available to verify location of Medical Gas leak and 26 valves to be replaced?  If so, when? Not currently planned
