NOTICE OF INTENT TO SOLE SOURCE Veterans Health Administration (VHA) Network Contracting Office (NCO)

7 Birmingham VA Health Care System Proposed Contractor:

Koffel Medical Supply NAICS: 621610 Description of Requirement The Department of Veterans Affairs, Birmingham VA Healthcare System, intends to award a firm fixed price sole source contract to Koffel Medical Supply for the continued provision and direct to patient delivery of proprietary Peritoneal Dialysis (PD) supplies and home dialysis support materials.

These materials include the specific Baxter Amia cycler connection systems, tubing assemblies, extension sets, and sterile components currently used by active VA home PD patients. The required supplies are clinically essential and must remain compatible with veterans existing PD equipment to ensure uninterrupted and medically safe home dialysis treatment.

Justification Koffel Medical Supply provides proprietary PD components uniquely compatible with the PD systems currently deployed in the homes of Birmingham VA s active PD patient population. These components cannot be safely substituted with another vendor s materials without requiring: full replacement of home PD equipment, retraining of all active PD patients, modification of clinical protocols, and risk of treatment interruption for medically fragile patients.

Market research, Notice of Intent posting, and technical review confirm that no other vendor can supply components compatible with the established system without requiring a clinically unsafe platform conversion. This action is authorized under 41 U.S.C. §3304(a)(1) as implemented by RFO 6.103 1, Only One Responsible Source.

Koffel Medical Supply is the only responsible source capable of supplying the proprietary PD supply system currently integrated into Birmingham VA s: clinical workflows, inventory management, patient support protocols, and home dialysis safety procedures. Transitioning veterans to any alternate PD platform would impose medical risk, continuity of care disruption, and unsafe treatment interruption, making a competitive acquisition unsuitable. Responses This notice is not a request for competitive quotes.

Interested parties may submit a capability statement demonstrating full, documented compatibility with the specific proprietary PD platform and consumable system currently in use by all Birmingham VA home dialysis patients.

Statements must demonstrate technical equivalence, interchangeability, and risk free patient transition, as required under RFO 6.103 1. Failure to submit evidence meeting all compatibility and continuity of care requirements may result in the Government proceeding with the proposed sole source award. The Government will not consider generic PD supply statements or proposals that would require changing patient equipment or altering clinical workflows. Capability statements must be emailed to:
