Description of Work, Appendix A Bath and Cn, LOS certification Instructions to Offerors RFIs go to Devan.Bertch@va.gov and pay special attention to Basis of Award section Solicitation Provisions and Contract Clauses Certifications/Representations When submitting the proposal: Offeror must complete and submit one (1) set of the SF1442 at time of Bid submission. Section 1-2 pdfs as described under Basis of Award in Instructions to Offerors Offeror shall submit a Bid Bond along with the bid.

Offeror shall submit EMR (Experience Modification Rate) rating with bid. Offeror must be registered in: www.beta.sam.gov; Certifications/ Representations must be completed. SDVOSB/VOSB offerors must be verified in www.vetbiz.gov at time of offer and award. Offerors must be registered as a small business under the applicable NAICS code noted in the solicitation. Offerors should have an account in Dun & Bradstreet and ensure that the account is up to date. Government may request additional financial information.

Proposals are due 1:00 PM 07/23/2024. Cutoff date for RFI s is 07/12/2024 at 4:00 PM. The Government intends to use the Lowest price technically acceptable source selection process in accordance with FAR 15.101-2. BE SURE TO SUBMIT DOCUMENTATION TO SATISFY SECTIONS 1-3 DESCRIBED UNDER BASIS OF AWARD.

Prior Experience (road construction) with a reference name/number, email from reference, OR CPARS Schedule fair amount of detail with a start date in August and Completion date in October or sooner Price Lowest price of those firms deemed technically acceptable Procedure for requesting information/clarifications: Offeror is responsible for bringing any blatant ambiguities in the documents to the Contracting Officers attention.

All questions must be in writing and emailed/sent to the Contract Specialist Devan.bertch@va.gov All technical questions will be answered by the Contracting Officer, in writing, via amendment to the solicitation. Amendments will be posted at: beta.SAM.gov An amendment may also be used to communicate other information (i.e., updated wage decisions, revised clauses, extended bid closing date, etc.). Offeror must acknowledge receipt of all amendments issued.

Offeror and Offeror s subcontractors are not permitted to contact Engineering Service for information regarding this project. To do so will be to your detriment as any answers you receive will not be applicable in a dispute with the Contracting Officer.

Additional Site Visits/Questions: This is the only scheduled site visit. No further site visits will be conducted. I have been notified that another COs name appears on the Milling and Patching Solicitation. This is a typo and all correspondence should go to Devan.Bertch@va.gov Be sure to include the Limitations on subcontracting form with each offer. There will be another site visit for Milling and Patching in Canandaigua at 1:30pm today (7/9/2024)
