NOTICE OF INTENT TO SOLE SOURCE IAW (in accordance with) FAR 6.302-1, The Department of Veterans Affairs, Providence, Rhode Island Healthcare System intends to award an option-year firm-fixed, sole source contract for ProCare Services to ensure uptime capability of the OR system to Stryker Communications, the sole manufacturer.

The proposed procurement is for services for which the Government intends to award to only one source under the authority of FAR 6.302-1 Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. 41 U.S.C. 3304(a)(1). All responsible sources that feel they should be considered qualified to provide the above-mentioned supplies may submit a response to this notice (by the response due date shown above) to kim.mccarthy@va.gov.

The information provided shall include detailed (and relevant) documentation to allow the government to perform adequate market research and make a proper determination. A determination by the government not to compete this proposed requirement based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct an unrestricted open market procurement. THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS.

THERE IS NO SOLICITATION AVAILABLE AT THIS TIME. REQUESTS FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. STATEMENT OF WORK Stryker - ProCare Services The Contractor shall provide all labor, personnel, equipment, tools, materials, supervision, and other items necessary to perform the onsite and remote services and support as outlined in this statement of work for the Stryker OR integration system currently installed at VAMC Providence, RI.

1. SERVICES/EQUIPMENT: The contractor shall provide all tools and equipment necessary to properly and safely perform preventative and maintenance services set forth to ensure uptime capability of the Stryker OR integration system which includes, but not limited to, the items listed below. This shall include, but is not limited, to all Personal Protective Equipment (PPE), tools, replacement parts, and authorized personnel which may be required for the work to be completed in a safe manner.

2. AREAS TO BE SERVICED: Providence VA Medical Center 830 Chalkstone Ave Providence, RI 02908 3. TASK FREQUENCY AND INSTRUCTIONS: Vendor shall provide all service equipment stated in the schedule A Service Coverage quote. POC for the PVAHCS will be Madison Gravel, Clinical Engineer, at 401-273-7100 x13562.

4. SPECIAL WORK REQUIREMENTS: None.

5. PROPERTY DAMAGE: The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, curbs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government, upon approval of the Contracting Officer.

6. ATTACHMENTS: None. 7. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall always wear visible identification while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions.

Smoking is prohibited inside any buildings or grounds at the VA. Cellular phones and two-way radios are not to be used within six feet of any medical equipment. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States (Federal) District Court, not a local district state, or municipal court.

8. GENERAL: Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. The A&A requirements do not apply, and a Security Accreditation Package is not required.
