STATEMENT OF WORK PROJECT OVERVIEW Accreditation Services to be provided using a standardized process by Commission on Cancer on behalf of the VA Boston Healthcare System.

A. GENERAL INFORMATION 1. Title of Project: Application for Accreditation by the Commission on Cancer.

2. Scope of Work: The contractor shall provide the resources and services necessary to review VA Boston s application for accreditation and provide feedback during a site visit at a later date to be determined.

3. Performance Period: The contractor shall deliver services Starting August 2024 and continue annually, unless otherwise directed by the Contracting Officer.

4. Managing Agency: The Associate Director Quality Management is managing this project and will be the contractor s principal point of contact regarding the execution of the contract s requirements. B. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below.

Deliverable 1: Execution - The contractor shall execute the services as described in this SOW.

Deliverable 2: Completion - The contractor shall provide all resources and services necessary for the delivery of Accreditation within the performance period listed above. DELIVERABLE DATE GOVT ESTIMATE EXECUTION of Application with site visit. August 2024 C. SCHEDULE OF DELIVERABLES The contractor s performance period dates will serve as the schedule of delivery for the deliverables specified.

D. GOVERNMENT RESPONSIBILITIES Government shall provide information to help develop designated topics and shall make available appropriate VA contacts as required by the work plan. The government shall complete and release required reviews and approvals in a timely manner and in accordance with the work plan.
