If you would like to submit quotation, follow the instructions in Section 3 of the solicitation, complete the required portions of the attached document, and submit it to the address shown on the Standard Form 1449 Amendment of Solicitation is done as per Continuation sheet of SF30, as per attached document. Continuation of SF30 1) Line item 7 is added for the DBA insurance on Section 1 - Point II.

Pricing (Base Year + Option Years) 2) Correction of table for Option Year 3 – Clin 001 is added by mistake twice (one as Line item 1 and the same was copied after Line 6 on the Solicitation document)
