The Defense Health Agency (DHA) intends to negotiate on a sole source basis in accordance with FAR part 13.106-l(b)(l). The proposed source is Allied Universal Technology Services, 8936 Western Way, Bldg. 6, Suite 11, Jacksonville, FL 32256. This acquisition is for NAS JAX Hospital-Facilities Management-Perimeter AMAG Access Control System. Services include furnishing all labor, materials, transportation for the installation of twentyfour (24) AMAG access controls to the hospital's perimeter. It is the contractor's responsibility to be familiar with the applicable FAR and DFARS clauses and provisions. FAR Clauses: http://acquisition.gov/browsefar DFARS Clauses: https://www.acq.osd.mil/dpap/dars/dfarspgi/current/ This is a new Fixed Price non-personal services contract for the installation of twenty-four (24) AMAG access controls to the hospital's perimeter as part of the NAS Jax security system. NAS JAX Hospital security team requires installation of access control on twenty-four (24) perimeter doors entering the hospital.

The doors requiring access control are not primary patient entry doors but are used by the hospital staff. These doors are currently unlocked manually by key as part of the beginning of day security process and re-secured each evening after duty hours by the same. Contractor shall be OEM authorized dealer, authorized distributor, or authorized reseller for the proposed software/hardware and equipment such that OEM warranty and services are provided and maintained by the OEM.

All software licensing, warranty, and services associated with the software/hardware and equipment shall be in accordance with OEM terms and conditions. Installation services include:

1. Install one (1) M2150-8DBC-DL-HSE 8 reader controller, nine (9) MTK15 Multi-Tech Reader with Keypad, Sngl, Blk and one (1) WIM8 Wiegand Interface Module for 8 read.

Location: BLDG 2080-Trash Chute rooms Room 1131, 1132, 2022, 3036, 3037, 5027, 6028, 7028, 8028 and AMAG panel located in room 2023. Included: Installation, Project Management, CAD and Engineering.

2. Install twelve (12) MTK15 Multi-Tech Reader with Keypad, Sngl, Blk and two (2) WIM8 Wiegand Interface Module for 8 read.

Location: Bldg 2080, Room 4097A-1 in and out reader, Room 4097A in and out reader, 4502 stairwell out reader, 4097-3 in and out reader, 4097-C front door in and out reader, 4097-C-2 in and out reader, 4018B in reader. Two AMAG panels located in Room 4035E each to have 1 WIM8 module added. 2 WIM8 modules in total.

Included: Installation, Project Management, CAD and Engineering.

3. Install three (3) MTK15 Multi-Tech Reader with Keypad, Sngl, Blk and two (2) PS12120F2 Battery, Sia, 12Vah, F2.

Location: Bldg. 2091 Front door, room 108 Warehouse Entry, Warehouse outside door.

Included: Installation, Project Management, CAD and Engineering.
