Equipment Specifications Requirements: 12 Full Power Tilt Exam Chairs Chair Glides/Chair Movers Instrument stand and consoles that are wheelchair compatible System functionality: Memory programming Cuts wasted movement and improves efficiency Dual-side operation controls to improve access to patient Adjustable headrests and armrests lock Power tilt Dual rotation locks Large hinged footrest Detached footswitch Software Vendor shall ensure all software operating on the device provided is supported by the original manufacturer at time of delivery.

For example, vendor shall not provide VA Saint Louis a device utilizing the Windows XP or Windows 7 operating system as they are no longer Microsoft supported. Proprietary software systems are acceptable so long as vendor supports the version provided to VA Saint Louis and it is the newest version at time of delivery. Access to all software shall be provided to VA Saint Louis in situations where re-installation is necessary.

Software shall be provided to VA Saint Louis with unlimited perpetual licensing for the supported life of the device. In cases where vendor insists on utilizing a licensing fee model this should be noted in the bid provided for VA Saint Louis consideration.

The Department of Veterans Affairs, Network Contracting Office 15 intends to award a sole source contract under the authority of 41 U.S.C 253(c) (1) FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements, to Corps Medical Supply. Corps Medical Supply, LLC is the only vendor able to work on the FX-920 Full Power TiltTM Examination Chairs and Reliance Chair Glide and the only one who produces the required parts as they are the Original Equipment Manufacturer.

The place of performance is for the Department of Veterans Affairs, Network Contracting Office 15, located at 915 N Grand Blvd, Saint Louis, MO 63106, in support of VISN 15 VA Health Care System at 1201 Walnut St, Kansas City, MO 64106 The NAICS code is 339113 Surgical Appliance and Supplies Manufacturing, with a size standard of 800 employees, and PSC 6515.
