Please note that there will be no further site visits scheduled, and no further RFI questions received. GENERAL NOTES: 1. CONTRACTOR SHALL COORDINATE ALL EFFORTS IN ACCORDANCE WITH ENFORCED INFECTION CONTROL RISK ASSESSMENT (ICRA) REQUIREMENTS. Spec Section 01 00 00 par. 1.8 Infection Control Risk Assessment (ICRA) Team.
Question: Could the VA kindly provide the VAMC s Infection Control Risk Assessment Guidelines for this project? An ICRA Permit will be required and will be provided by the ICRA Nurse once the contractor has mobilized. Sample Permit is attached. At a minimum, ICRA Permit will require poly plastic zip door at entrance, Anteroom inside the OR with negative air inside Anteroom, poly plastic to seal/cover all doors, cabinets, booms, and lights. Seal air diffusers. Install temporary filters on return air vents. Additional negative air in main OR construction area. 3. Spec Section 01 00 00 1.1 GENERAL INTENTION.
Question: Can the Prime Contractor s Superintendent also serve as the Prime Contractor s SSHO? Yes. 4. Spec Section 01 00 00 1.6 OPERATIONS AND STORAGE AREAS.
J. Utilities Services: Maintain existing utility services for Medical Center at all times. Provide temporary facilities, labor, materials, uninterrupted services.
Question: Could the VA kindly provide if any which temporary services the GC will be responsible for during shutdowns? GC would provide own labor and materials for any specialty power/equipment needed for construction. VA would identify power source.
5. Spec Section 01 00 00. Question: Can the PM perform dual role as the QCM as well? Yes 6. Confirming all work will take place at night, and what the working hours will be. Work can take place during the day, with exception to outages. Outages will need to be afterhours and can begin at 6:00pm generally, barring any unforeseen OR emergency cases. 7. During the site walk OR #5 was being worked on. The space was protected plastic and a plastic zipper door.
The Spec section calls out for plywood or gypsum board partitions. What level ICRA Plans will this job be required to follow. See #2 response. 8. Will a negative air machine with gauge be required for the duration of project? Yes. Gauge required to be digital type for Anteroom and OR (construction area).
