STATEMENT OF NEED Title of the project:

Inserter/Folder Mailroom Background: The mailroom currently has a folder that was purchased May of 2011 and reached the end of its life expectancy two years ago. It has started to experience paper jams and clogging. This equipment is used to fold patient letters and is in high use throughout the year. Tasks and deliverables: This is a brand name OR equal requirement for 1 each Formax 6306 Folder. Salient Characteristics include:
