STATEMENT OF NEED Title of the project:

**Inserter/Folder Mailroom Background:** The mailroom currently has a folder that was purchased May of 2011 and reached the end of its life expectancy two years ago. It has started to experience paper jams and clogging. This equipment is used to fold patient letters and is in high use throughout the year. Tasks and deliverables: This is a brand name OR equal requirement for 1 each Formax 6306 Folder. Salient Characteristics include:

**Speed:** up to 3,800 pieces per hour Duty Cycle: up to 40,000 pieces per months Standard Feeder: up to 325 sheets each, 20 bond High-Capacity Feeder: (standard 2F, Special 2F, Standard 2FP models) up to 725 sheets, 20 bond Envelope Hopper: up to 400 envelopes, top- loading High-Capacity Vertical Stacker: up to 500 filled envelopes Sheet Size: 3.5 - 14 H x 5 -9 W Programmable Jobs:
