1. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested, and a written solicitation will not be issued.

2. The solicitation number is W912L6-25-Q-A008 issued as a Request For Quote (RFQ) in accordance with the requirements of Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, using the procedures of FAR Part 13, Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2025-04 dated June 11, 2025. The FAC and FAR are available at https://www.acquisition.gov.

3. This solicitation is a Small Business Set-Aside with the North American Industry Classification System (NAICS) code 621111, Offices of Physicians (except Mental Health Specialists) with a standard size of $16.0M and Product and Service Code (PSC) Q403, Medical—Evaluation/Screening.

4. The Government will not reimburse any costs incurred or associated with in relation to the preparation of a quote for this solicitation, W912L6-25-Q-A008.

5. It is the Government’s intent to award a Lowest Price Technically Acceptable (LPTA) Firm Fixed Price (FFP) contract for the Oklahoma Army National Guard (OKARNG) without discussion.

6. Contractor shall provide all services as stated in the Performance Work Statement (PWS) as an attachment to this solicitation.

7. Delivery and Acceptance will be FOB Destination, per the delivery location stated in the PWS.

8. Provision 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services applies to this acquisition, to include any addenda to the provision. Provisions and Clauses are attached to include the technical evaluation criteria for award.

9. The Offeror is required to have a current and active registration in the System for Award Management (SAM) website, https://sam.gov at the time the quote is submitted and not in a “pending” status, as well as no exclusions.

10. Questions shall be submitted via email no later than July 31st, 2025, by or before 10:00 a.m. CST to MacKenzie Boyd at mackenzie.a.boyd.civ@army.mil.

11. Quotes shall be submitted via email no later than August 12th, 2025, at 10:00 a.m. CST to MacKenzie Boyd at mackenzie.a.boyd.civ@army.mil. It is the Offeror’s responsibility to ensure all questions and quote documents are received prior to the close date and time. No phone calls.
