UPDATE - Perferred method to SUBMIT QUOTE is via email to the Primary Point of Contact (POC) identified in the Contact Information section of this notice. This Award will be made using FAR 13 Simplified Acquistion Procedures in conjunction with FAR Part 12 Acquistion of Commercial Products and Commercial Services. This is a REQUEST for QUOTEs (RFQ). In response to the successful Quote, Government intends to issue a Purchase Order (PO) for Annual Industrial Hygiene Surveys at Naval Air Station Jacksonville, Florida.

Contract Award will follow Government receipt of PO signed by Offeror to signal acceptance. Contract type will be Firm Fixed Price (FFP). Period of Performance (PoP) is one (1) base year and four (4) Option Years. General information This is a combined synopsis/solicitation for commercial items prepared In Accordance With (IAW) the format in FAR Subpart 12.6 “Streamlined Procedures for Evaluation and Solicitation for Commercial Items”, as supplemented with additional information included in this notice.

Cost of Quotes: Expenses incurred in the preparation of quotes in response to this RFQ are the vendor’s sole responsibility and not reimbursable by the Government. INSTRUCTIONS TO OFFERORS In order to be eligible for award Quotes shall include Unit Price for each CLIN and Total Price, Company Name and CAGE Code, Company Point of Contact (name, phone number and email address), and a Capability Statement. BASIS FOR SELECTION Pursuant to FAR 12.602 contract award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106. Offeror’s submissions will be evaluated based upon the following:

Technical: quote must be rated as technically acceptable to be eligible for award. To be deemed technically acceptable, the salient characteristics listed in the solicitation must be met.

Price: Award will be made to the lowest price technically acceptable offer from an Offeror with an active SAM.gov account (System for Award Management (https://www.sam.gov)) and an acceptable Supplier Performance Risk System (SPRS) rating (https://www.sprs.csd.disa.mil).
