Purpose of Amendment 0005: The purpose of this amendment is to respond to the RFIs 33 through 35 and incorporate revised section L to remove the requirement of a Dun and Bradstreet Report. The SF30 which addresses the RFIs is issued as actual Amendment 0004. The response date remains unchanged, May 09, 2024, at 2:00 PM AZ Time.**Purpose of Amendment 0004: The purpose of this amendment is to respond to the RFIs and incorporate Exhibit N - 2023 Bituminous Specifications.

The SF30 which addresses the RFIs is issued as actual Amendment 0003. As a result of the RFIs the following sections have been revised: B-Supplies or Services and Price/Cost, C-Description/Specifications/Work Statement, E-Inspection and Acceptance, L-Instructions, Conditions, And Notices to Offerors, M-Evaluation Factors for Award and Revised, and Exhibit M - Exhibit Line Item Pricing Paving IDIQ.

The following are being provided in word format as applicable: Revised Exhibit A - Offeror Team Composition and Management, Exhibit B - Experience ? Offeror, Exhibit C - Past Performance Questionnaire (Form PPQ), Exhibit D Safety Data, Revised Exhibit E - Financial Questionnaire, and Exhibit F - Surety Form, Revised Exhibit I - Wage Determination AZ 20240042 (*PDF). The response date has been extended from April 26, 2024, to May 09, 2024 at 2:00 PM AZ Time. See SF30 attachments for additional details.

Please note that amendment 0003 within PIEE was done to revise the closing date within the PIEE system to April 26 as Amendment 0002 only posted the SF30 and did not update the systems response date.
