2) Update Sections (B.3.2 - Bi-Weekly Retention Amounts), (B.6- GROUP LIFE INSURANCE RATES); and (B.7-ADMINISTRATIVE RETENTION AMOUNTS) (See continuation page of Block 14) on pages 2 to 6 and Update Section (J)(adding Claims reports, Detailed Census list for Health Insurance. and Detailed Census list for Life Insurance (See continuation page of Block 14) on pages 7 to 26; and 3) Add Attachment No.1 - "Questions & Answers " 4) CERTIFICATION REGARDING COMPLIANCE WITH APPLICABLE FEDERAL ANTI-DISCRIMINATION LAW (Page 27).
