BPA for various Endodontic Supplies for the Dental Dept. at the KHC

The Indian Health Service, Kayenta Service Unit – Kayenta Health Center, located in Kayenta, AZ 86033 has an upcoming requirement for a Blanket Purchase Agreement (BPA) to order various Endodontic supplies for root canal therapy to be ordered on an as needed basis. The projected Period of Performance is for a one-year contract; duration of 12 months as a Base plus Two (2) Option Years from date of award.
