Companion Care Services for Southern Arizona VA Health Care System (SAVAHCS)

Contractor shall provide non-personal Companion Care Services to eligible beneficiaries authorized by the Southern Arizona VA Health Care System (SAVAHCS). Contractor shall provide all necessary personnel, equipment, transportation, and supplies associated with the full range of Companion Care Services within the terms, conditions, and provisions of this contract. The VA beneficiaries to be cared for under this contract, as referred by the SAVAHCS, shall receive the Companion Care Services after post procedure.

Prior to the beneficiary release from the VA hospital, SAVAHCS will screen, verify, and refer the VA beneficiary to the contractor for companion care services. The VA providers will verify that the beneficiary no longer requires medical supervision but requires caregiver observation.
