NOTICE OF INTENT TO SOLE SOURCE: The Department of Veterans Affairs Network Contracting Office 22 intends to award a sole source, firm-fixed-price, contract to Steris Corporation for Preventative and Corrective Maintenance on Steris Sterilization Equipment to the Biomedical Engineering with Phoenix Veterans Healthcare System, Carl T. Hayden Medical Center located at 650 East Indian School Road, Phoenix, AZ 85012. This contract action is for services for which the Government intends to solicit from only one source under the authority of FAR 8.405.6 Only One Source.
Interested parties may identify their interest and capability to respond to the requirement or submit proposals. The North American Industry Classification System (NAICS) for this requirement is 811210 Electronic and Precision Equipment Repair and Maintenance with a small business size standard of $34 million.
THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE QUOTATIONS The Department of Veterans Affairs, Phoenix VA Medical Center intends to award a firm fixed price, single source purchase order to Steris Corporation. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
The Government will not be responsible for any costs associated with the preparation of responses to this notification. All inquiries and submittals must be sent via email to Doreen Leah Masterson, Contracting Specialist, at Doreen.Masterson@va.gov. Telephone inquiries will not be accepted. Required Services The Contractor shall be responsible for preventive maintenance in accordance to manufacturer s recommendations The Contractor shall be responsible for repairs required during preventive maintenance.
The Contractor shall be responsible for corrective maintenance. The Contractor shall be responsible for all labor. The Contractor shall be responsible for all travel. The Contractor shall be responsible for all calibration and testing equipment. Phone response time: 4 hours minutes On-site response time: 48 hours The Contractor shall notify the COR to schedule a time and their intent to come on-site. The Contractor shall verify that the equipment that was serviced is fully functional before checking out.
The Contractor shall deliver a hard copy or electronic copy of a Field Service Report that delineates the service performed and confirms quality assurance tests results to Agency, see paragraph 4. The Contractor shall inform the COR prior to adding or removal of equipment along with providing documentation for both equipment added or removed. Service technicians shall be certified to perform services on the enclosed specified equipment. Contractor shall provide documentation/copy of certification.
Waste Removal: The contractor is responsible to remove and properly dispose of all waste related to the services provide on-site. Failure to carry out the above procedures shall be considered as a failure to document the service visit and shall constitute a failure to perform. This contract shall provide for the addition and removal of equipment covered in the scope of services. Equipment Inventory Listing to be Supported: L/I MANUFACTURER MODEL i.
