Sources Sought The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service-Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Business Manufacturers, other Small Businesses interested and capable of providing items requested. The intended contract is a firm-fixed price contract.
A SBA Non-Manufacturer Rule Waiver will not apply if this requirement is set-aside for small business. It does apply to all other socioeconomic small business categories. VA Puget Sound Health Care System is procuring Preventative Maintenance of TWO (2x) CONSTELLATION VISION SYSTEMS, to include all parts and labor necessary to repair/replace the listed equipment for all damages incurred to the devices during the contracted period. Potential contractors shall provide, at a minimum, the following information to Denise.Patches@va.gov.
1) Company name, address, and point of contact, phone number, e-mail address, and SAM UEI.
2) Please identify your company’s size in comparison to the anticipated North American Industry Classification System (NAICS) code 811210 Electronic and Precision Equipment Repair and Maintenance. This notice is to determine the marketplace for this specific requirement. The largest a firm can be and still qualify as a small business for Federal Government programs is no more than 1,000 Employees.

Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application in the System for Award Management (SAM).
4) Any Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. 5) Only authorized representatives/providers of the manufacturer will be considered. Equipment must be new, no used or refurbished equipment accepted.
Please provide a proof of authorized dealer or reseller letter from manufacturer. ) If you are the manufacturer, do you have any designated or authorized distributors? If so, please provide their name, telephone, point of contact and size status (if available). 6) This is a “brand name or equal” requirement . As such, the salient characteristics reflect the physical, functional, or performance characteristics that “equal” products must meet to satisfy the Government’s needs.
To be considered for any potential solicitation, offers of “equal” products, including “equal” products of the brand name manufacturer, must: Identify the manufacturer name (OEM) and manufacturer part number, as well as any warranty information. Please provide a courtesy quote in response to this request to evaluate price reasonableness for any set-aside determination. Please provide the country of origin. It is the intent of the VA to use the information gathered for market research purposes only.

Point of Contact: Denise Patches Contract Specialist Denise.Patches@va.gov STATEMENT OF WORK (SOW) – PREVENTIVE MAINTENANCE 1. Contract Title:
Constellation Vision System Gold Service Contract 2. Background: The Seattle VA Medical Center has the need for a preventive maintenance coverage and sustainment for their Constellation Vision System. The OR has two Constellation VISION equipment in the OR that requires a contract for coverage rather than micro-purchases for PM and coverage. The system also has a laser component that if broken would require a purchase well over any purchase card limit to repair.
3. Scope:
a. The Contractor shall comply with Federal, State, and Local Laws, plus any Federal Regulations as applicable to the performance of this contract.
b. The Contractor shall only take direction from the Contracting Officer (CO), or the Contracting Officers Representative (COR) in all matters concerning this contract.
c. The Contractor is perform Preventative Maintenance and repairs on the Support Holmium laser. Contractor shall maintain qualifications throughout entire contract period. Qualification includes but not limited to the certification of all repair persons and repair facilities by the OEM as being trained / qualified to perform required repairs. The Government shall maintain the right to seek proof of qualification prior to award and anytime during contract period of performance.
4. Specific tasks:
a. Preventative Maintenance of TWO (2x)
CONSTELLATION VISION SYSTEMS, SN:1302174701X & SN:
1803354601X: to include all parts and labor necessary to repair/replace the listed equipment for all damages incurred to the devices during the contracted period.
b. The contractor shall furnish and replace parts to meet equipment specifications. The contractor must have ready access to all replacement parts. Contractor shall maintain a sufficient stock of all parts (meeting manufacture's design and specification) needed for repair and maintenance of the equipment or be able to secure needed parts within two (2) calendar days. All parts supplied shall meet manufacturer approved specifications. The contractor shall use new parts only.
c. Report will be acceptable electronically or paper format stating service provided and outcome of repaired equipment.
d. The C&A requirements do not apply, and a Security Accreditation Package is not required.
5. Government Furnished Property: Government furnished property will not be provided to the contractor 6. Performance Monitoring: The tasks are well defined and can, therefore, be easily monitored by the COR, who shall represent the Government for this process. Contractor performance will be monitored, and the work verified by the COR. The COR will assure contractor quality by routine inspections and monitoring of work while work is being performed.
7. Security Requirements: The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required.
8. Place of Performance: Work will primarily be performed at the PSHCS, Seattle Campus, Building 100 O.R. Dept., 1660 S. Columbian Way, Seattle, WA 98108. 9. Period of Performance (POP):
